Most Clinics Are Competing for the Wrong Patient

2026-08-09 · 5 min · by Vicente De Los Mozos

I have talked to 21 clinics across 7 countries this year. The pattern barely changes.

World-class surgeons. Real credentials. Equipment that matches anything in Miami. And a calendar filled with local patients being fought over on price.

Meanwhile there is someone in Houston or Toronto or Madrid who wants that exact procedure, would pay several times what the local patient pays, and cannot find them.

It is not a quality problem

Not because the clinic is not good enough. Because nothing about how it presents itself says "international destination." It says "clinic in my city."

That is a positioning problem. And positioning is cheaper to fix than anything else on the list.

What the ones who make the jump actually do

They stop describing themselves the way every other clinic does

"Quality plastic surgery" is not a position. It is what everyone says. If your description would survive being copied onto a competitor's website unchanged, it is not doing any work.

They build for a patient who has never been to the country

That means answering in that patient's language, at that patient's hour, before they have decided which country they are flying to. The decision window for medical travel closes long before most clinics have replied.

They treat the first conversation as a decision, not an explanation

If a consultation is where you explain what you do, you have already lost the people who compared you to two other clinics last night. The consultation should be where someone decides, which means the explaining has to happen before it.

What these patients are actually looking for

The patients traveling are not looking for cheap. They are looking for good, and good that happens to cost less than home. Those are different buyers and they respond to different things.

Competing on price attracts the first kind. Almost nothing about a discount reassures somebody who is boarding a plane for surgery.

The clinics that win them will not be the best surgeons. They will be the ones who were findable, understandable, and answering.
